Berryhill Animal Hospital Boarding Agreement

Vaccination Policy
ALL PETS boarding at Berryhill Animal Hospital are required to be currently vaccinated. For Dogs: RABIES, DISTEMPER, PARVO and a 6-month BORDETELLA. For Cats: RABIES, FELINE DISTEMPER, LEUKEMIA vaccine. If your pet is vaccinated at another facility, you must provide written verification. If documents are not presented at admission, we will phone or have your pet’s records faxed to us by your regular Veterinarian. Long-Distance telephone charges may be added to your charges if we need to call out of state. If your pet’s vaccinations are not up to date a Doctor at Berryhill will provide an examination and the necessary vaccinations and the fees will be added to your charges.

Policy on Fleas or Other Parasites
Your pet will be inspected for fleas/ticks/mites upon admission and prior to discharge from Berryhill Animal Hospital. If any fleas/ticks/mites or intestinal worms are observed, we require that your pet be bathed and/or treated for these parasites prior to or while boarding at our facility. If you wish, we can perform those services while boarding as a preventative measure.

Monday through Friday 7:00 am – 6:00 pm
Saturday 8:00 am – Noon
Sunday Courtesy Pick-up 4:30 pm - 5:00 pm (please note: All services must be pre-paid at time of check-in for Sunday pick-up)

Boarding Agreement
Berryhill Animal Hospital will use all reasonable precaution against injury, escape or death of my pet. The clinic and staff will not be held liable for any problems that develop provided reasonable care and precautions are followed. I understand that if a medical problem is discovered, Berryhill Animal Hospital will telephone the owner or emergency contact to discuss the problem and determine the best course of action to be taken. If the owner or emergency contact cannot be reached the attending veterinarian has my permission to take the necessary steps to diagnose and treat in accordance with current medical standards. In case of emergency, measures to preserve and stabilize vital function shall be taken immediately. I assume full responsibility for lodging, veterinary services and any other expense incurred while my pet is boarded. I verify that I have read and fully understand and agree with the terms of the agreement. I affirm that I am at least 18 years of age and I am the owner of the pet described below.

We provide water bowls, blankets and quality commercial dry pet foods unless you provide these items. As a courtesy to you we will wash your pet’s blankets and beds that you provide, time permitting. However we are not responsible for damages to them during washing, drying or your pet’s chewing
